Care coordinator
£13–£15 per hour, depending on experience and hours
Responsibilities:
Coordinate and schedule care visits to ensure people receive consistent, timely, and person-centred support in line with their care plans.
Act as a key point of contact for people we support, their families, care staff, and external professionals, ensuring clear and respectful communication.
Allocate care staff based on skills, experience, availability, and compatibility, promoting continuity and positive relationships.
Maintain accurate, up-to-date care schedules, responding promptly to changes, absences, and emergencies.
Support the development, review, and updating of personalised care and support plans in line with assessed needs and preferences.
Monitor the quality of care delivery, escalating concerns appropriately and contributing to continuous improvement.
Work closely with the management team to support compliance with Care Inspectorate requirements, policies, and procedures.
Ensure records are maintained accurately and confidentially, in line with GDPR and organisational standards.
Support care staff with day-to-day coordination, guidance, and problem-solving.
Participate in on-call or out-of-hours support where required, in line with service needs.

Home Care Worker
£12.65 per hour

Support people with daily living activities, including personal care, medication support as trained, meal preparation, and mobility.
Promote independence, dignity, and choice, supporting people to do as much as they can for themselves.

Follow agreed routines, preferences, and outcomes, recognising what matters to each individual.
Observe, record, and report any changes in health, wellbeing, or behaviour promptly and accurately.
Maintain clear, accurate, and confidential records in line with company policies and GDPR.
Work collaboratively with colleagues, families, and professionals to ensure consistent, high-quality care.
Follow infection prevention and control procedures and health and safety guidance at all times.
Attend required training, supervision, and team meetings, supporting ongoing learning and development.
