Risk and review
Care plans and risk assessments are reviewed when needs change. This may include mobility, nutrition, medication, skin integrity, cognition, falls risk or home safety.
Medication oversight
Where medication support is agreed, staff follow the care plan and recording process, including MAR entries and escalation for refusals, omissions, PRN concerns or unexpected changes.
Communication with families and professionals
With appropriate consent and in line with confidentiality and UK GDPR, concerns may be shared with family members, health professionals, social work teams or emergency services where needed to support safer outcomes.
