Safe systems, clear escalation and adult protection
Good care at home depends on safe systems as well as kind support. Shona Care Ltd expects concerns to be recorded clearly, escalated appropriately and reviewed for learning.
Safeguarding
Staff are expected to speak up about adult protection concerns, unexplained changes, neglect, abuse, medication concerns, environmental risks or any issue that may affect safety or wellbeing.
Escalation
Concerns may be escalated to managers, families, health professionals, social work teams, emergency services or the Care Inspectorate where appropriate.
Learning culture
Complaints, incidents, near misses and feedback are reviewed so practice can improve. Duty of Candour and Care Inspectorate notification requirements are considered when relevant.
Medication and care-record governance
- Clear recording of visits, changes and concerns.
- MAR and medication support checks where medication support is part of the care plan.
- Manager review of refusals, omissions, PRN concerns and unexpected changes.
- Supervision, spot checks and staff reflection to strengthen safe practice.
